Job Description

The Behavioral Health Home Hospital (BHH) Research Social Worker/Case Manager will support an innovative care delivery model Behavioral Health Home Hospital. The program offers a subset of patients an alternative to brick-and-mortar inpatient psychiatric hospital admission in which patients can receive highly specialized psychiatric-care services in their home. The BHH Research Social Worker/Case Manager will provide quality clinical services to patients in various settings in the community. Patient care is delivered within organizational philosophy policy and standards of nursing and community health practice. The responsibilities are performed in accordance with Brigham and Women s Faulkner Hospital s policies and standards of practice utilizing thorough and timely electronic documentation and interdisciplinary communications. The BHH Research Social Worker/Case Manager will collaborate with other professionals to provide clinical services to patients/families that address psychosocial environmental age specific and cultural issues in order to maximize emotional social and physical well-being and the effective use of health care and community resources. They utilize clinical social work techniques and theory to provide consultation assessment treatment interventions and advocacy for BHH patients and their families. The BHH Research Social Worker/Case Manager can expect to see patients in person in their homes or space they are residing in during their hospitalization or remotely. Additionally the BHH Research Social Worker/Case Manager can expect to have up to 4 patients on service at a time. The BHH Research Social Worker/Case Manager will be providing hospital/inpatient-level treatment to patients in the community. This is an ideal role for someone interested in a temporary position and working on a research trial that will help broaden the treatment opportunities for patients with active acute-psychiatric conditions. This position will begin on 1/5 when the trial begins - the schedule will depend on caseload but anticipate about an hour a day.
