This role provides essential psychiatric and medical support health education and medication management to individuals living with serious mental illness and co-occurring substance use disorders.Working under the supervision of the ACT Registered Nurse and Psychiatrist/MD the LPN will deliver care in participants homes and community settings monitor health assist with medications and support the interdisciplinary team in treatment planning and care coordination.Responsible for on-call shifts monitoring and responding to after-hours calls to the program emergency phone.
