LPN, Acute (Day Shift) at Gothenburg Memorial Hospital

Position SummaryGothenburg Health is seeking a full-time Acute Care Licensed Practical Nurse/Licensed Practical Nurse-Certified. This role provides direct patient care meeting both psychological and physical needs of the patients in accordance with physicians orders nursing and department guidelines. The LPN/LPN-C contributes to the continuity and quality of care which fosters the best interest and well-being of all patients and their families.Key AccountabilitiesConduct and document a thorough assessment of each patients medical status upon admission and throughout the patients course of treatment for continuity of care.Assist the RN with emergency and outpatient care which may include accompanying a patient on a transfer to another facility for continuity of care.Assists in developing and implementing an individualized treatment plan for each assigned patient to ensure quality clinical outcomes.Assume when assigned the duties of medication nurse or floor nurse to provide for efficient workflow.Provide regular patient status updates to appropriate personnel to ensure transfer of information.Complete required documentation in an accurate and timely manner for up-to-date charts and records.Minimum QualificationsGraduate of an accredited school of nursing.Current License Practical Nurse (LPN) licensure in the state of Nebraska.Current BLS certification within 6 months of hire.About Gothenburg HealthGothenburg Health is a critical access hospital and clinics located in Gothenburg Nebraska.
