Job Summary:

The LPN will provide high-quality, patient-centered care to our residents, supporting their unique needs and preferences. Responsibilities will include, but are not limited to:

• Administering medications and treatments as prescribed by physicians
• Monitoring and reporting patient vital signs and changes in condition
• Providing essential care services, such as bathing, dressing, and personal hygiene
• Assisting with daily living activities, including feeding, toileting, and ambulation
• Collaborating with the interdisciplinary care team to develop and implement individualized care plans
