Responsibilities:
Provide direct care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with residents, families, and healthcare teams
Conduct routine assessments and evaluations to ensure resident health and well-being
Maintain accurate and up-to-date records and reports
Participate in interdisciplinary team meetings and contribute to care plan development
Demonstrate a commitment to resident safety and prevent adverse events
Requirements:
Completion of a state-approved training program
Current LPN license (pass NCLEX-PN exam)
BLS certification
