At West Park Rehabilitation and Nursing Center, we are committed to Revolutionizing Care Delivery™ and providing exceptional patient-centered care to our residents. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals.

About the Role:

As an LPN with our team, you will play a vital role in delivering high-quality, patient-centered care to our residents. Your responsibilities will include:

  • Assisting with daily living activities, such as bathing, dressing, and grooming

  • Administering medications and treatments as prescribed by our medical team

  • Monitoring vital signs and reporting any changes to our nursing staff

  • Assisting with wound care and other medical procedures as needed

  • Maintaining accurate and detailed records of patient care and treatments

  • Collaborating with our interdisciplinary team to develop and implement individualized care plans

Responsibilities:

  • Provide direct patient care, including administering medications, performing wound care, and providing compassionate support to our residents

  • Collaborate with our interdisciplinary team, including physicians, therapists, and other healthcare professionals to develop and implement individualized care plans

  • Maintain accurate and detailed records of patient care and treatments, including medication administration and vital signs

  • Participate in ongoing education and training to stay up-to-date on the latest medical practices and technologies

  • Contribute to a positive and supportive work environment by fostering open communication and teamwork
