Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team. As a valued member of our healthcare team, you will play a crucial role in delivering high-quality patient care to our residents. In this role, you will work collaboratively with other healthcare professionals to provide exceptional care and attention to our patients' needs.

Responsibilities

• Provide direct patient care to residents in accordance with established care plans and protocols
• Administer medications, perform wound care, and provide basic wound care to residents as needed
• Monitor and report any changes in patients' conditions to the nursing staff and healthcare providers
• Collaborate with other healthcare professionals to develop and implement individualized care plans for residents
• Maintain accurate and up-to-date records of patient care, including progress notes and medication administration records
• Participate in quality improvement initiatives to ensure high-quality patient care

About This Opportunity

Working Schedule: Evening shift, Monday to Friday, 3:00 pm to 11:00 pm
