Employment Type:Part timeShift:Day ShiftDescription:Hospice Homecare LPN - Per Diem Saratoga&Washington CountiesWEEKDAYS 8am to 4pm The Community Hospice has a wonderful opportunity for a Per Diem LPN to workas a member of our Hospice Home Care Team caring for patients and families in both the homecare and nursing home settings. Primary region to cover is Saratoga and Washington Counties. From comprehensive training to providing necessary protective equipment were committed to creating a secure environment for you as you deliver exceptional care in our clients homes.What you will do:Coordinate closely with ourRN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and familiesThis is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs Aides medical directors with specialized expertise in end of life care social workers chaplains complementary therapists and volunteers).Training provided in end of life care. What you will need:Valid NYS LPN LicenseMinimum 1 year LPN experience requiredPrior hospice/ home care experience preferred but not requiredStrong computer skillsOrganization and communication skills and ability to work as part of a teamCommunity Hospice provides a thorough orientation preceptors and a supportive work environment. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
