About the Role:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our night shift team at our Smethport, Pennsylvania, facility. As an LPN at Bradford Manor Nursing and Rehabilitation, you will play a vital role in delivering exceptional patient care and contributing to our mission of providing high-quality services to our patients.

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, dressing wounds, and performing vital signs checks

• Develop and implement individualized care plans for patients, in collaboration with the interdisciplinary care team

• Maintain accurate and detailed records of patient care and progress

• Collaborate with other healthcare professionals to ensure seamless transitions of care and effective communication

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Current LPN license in the state of Pennsylvania

• Basic Life Support (BLS) certification
