About the Role:

As an LPN on our team, you will have the opportunity to work in a fast-paced and dynamic environment, providing direct patient care and support to our residents. Your responsibilities will include:

  • Providing and administering medications, treatments, and therapies as prescribed by physicians and as outlined in our nursing protocols

  • Conducting physical and vital sign assessments, documenting patient information, and reporting changes to the healthcare team

  • Developing and implementing individualized care plans in collaboration with the interdisciplinary team

  • Assisting with daily living activities, such as bathing, dressing, and grooming

  • Maintaining a safe and clean environment for our residents, adhering to infection control protocols and adhering to policies and procedures

Requirements:

  • Current LPN licensure in the state of Illinois

  • High school diploma or equivalent required; associate's degree or higher preferred

  • Minimum of 0 years of experience in a healthcare setting; prior experience as an LPN a plus
