Social Worker Naturally Occurring Retirement Communities (Fresh Meadows) at Elder Care

Position Summary:Provides assessment services coordination and counseling to geriatric clients eligible for receiving multiâservices in the community and in general aides clients in managing life situations; e.g. budget medical care shelter recreational and educational activities social services and health programs.Principal Responsibilities:Conduct intakes provide accurate information and referrals.Screen and assist clients with applications and re-certifications for entitlements and benefitsAssess clients in their home hospital or other institutional setting as appropriateAdvocate on behalf of clients and arrange for provision of services; develop implement and follow up on care plansProvide supportive counseling to clients as neededMaintain ongoing communication with client family members and other collaterals as appropriateMaintain complete and accurate case notes utilizing the programs data system; prepare required documentation in case file within organizational time framesMaintain and report on statistics according to Selfhelp standards utilizing the programs data system and other forms of record keeping as needed.Actively participate in supervision and staff meetings and share any emergent issues with supervisor in a timely mannerFacilitate and assist at social recreational and other group activities and eventsSupervise social work students (if applicable)Adhere to organizations standards by completing annual mandatory trainings in a timely manner on topics including but not limited to Sexual Harassment Prevention Training IT Security Awareness Compliance OSHA HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.Adheres to the organizations policy in regards to absenteeism and appearance and health and safety standardsSalary Range: $60000 - $62400 per year commensurate with experienceJob Competencies & Minimum Qualifications:Bachelors required; MSW preferredWorking knowledge of Microsoft Office Suite and other technologyExcellent communication and listening skillsExcellent customer service skills with a focus on treating clients with respect and dignityAble to multi-task and work independently with a great attention to detailWorking Conditions/Physical DemandBusiness office environment with phone and computer use travel via public transportation. This position may have the ability to work from home 2-3 times per week.
