MDS Coordinator (RN/LPN) at Scenic Wellness Nursing & Rehabilitation

We’re on the lookout for a detail-driven MDS Coordinator (RN or LPN) to join our amazing team at Scenic Wellness & Rehabilitation! If you’re passionate about accuracy, teamwork, and quality resident care, this is your place to shine.

What You’ll Do

• Oversee and complete the MDS and care management process from admission to discharge

• Work closely with the Interdisciplinary Team to ensure compliance with state and federal guidelines

• Maintain accurate documentation and data entry in MDS software

• Participate in meetings to review resident care, assessments, and quality measures

• Train and support nursing staff on MDS processes and PPS/Medicaid requirements

• Ensure care plans reflect residents’ current needs and changes

What You’ll Need

• RN or LPN license in Missouri (required)

• 2+ years of experience in a skilled nursing facility (preferred)

• Strong knowledge of MDS 3.0, Medicare PPS, Medicaid Case Mix, and OBRA regs

• Attention to detail and a heart for resident care
