Position Summary

We are seeking a Licensed Primary Care Physician (Family Medicine, BC/BE) to provide exceptional outpatient care while supporting clinical leadership initiatives. This role combines hands-on patient care with clinical supervision, quality improvement, and leadership responsibilities.


Key Responsibilities

  • Lead Provider Meetings and other meetings in place of the CMO as needed.
  • Cover CMO responsibilities during absences.
  • Assist the CMO in monitoring clinical performance metrics (HEDIS, STAR, UDS, etc.) via chart audits and reports.
  • Evaluate and treat patients at clinic sites, including acute, chronic, and preventive care.
  • Perform in-office procedures such as incision and drainage, arthrocentesis, nail removal, skin biopsy, and cryotherapy.
  • Maintain productivity standards of 22 patients per day.
  • Ensure accurate, thorough, and timely documentation; lock progress notes within 48 hours.
  • Review and sign off on lab results, diagnostic reports, and consult notes promptly.
  • Initiate and manage specialist referrals and e-consults, providing whole-person care.
  • Serve as primary care provider for a patient panel within a collaborative care team.
  • Clinically supervise Advanced Practice Providers (APPs), including new provider chart reviews and ongoing supervision.
  • Collaborate with Medical Assistants and participate in care team efficiency improvements.
  • Participate in team huddles, Provider Meetings, All-Staff Meetings, and Continuous Quality Improvement activities.
  • Provide compassionate care for patients with mental health, substance use, or social challenges.
  • Complete waiver training for Medication Assisted Treatment (MAT) if providing care for adult patients with substance use disorder.
  • Assist with community health screenings and outreach events.
  • Maintain active enrollment in Medicare, Medicaid, and other payor programs.


On-Call Expectations:

  • Rotating on-call coverage consists of carrying a phone for one week approximately every 2 months.
  • This is phone-only coverage, with no on-site requirement.
  • Periodically work occasional Sunday shifts to support operational needs.


Qualifications & Requirements

  • Licensed to practice medicine in California with an active DEA license.
  • BC/BE in Family Medicine only. Direct experience in Family Medicine is required; years of experience are less important than demonstrated passion, commitment, and alignment with serving an underserved and homeless population.
  • Residency Consideration: Recent residency graduates considered only if they served as Chief Resident.
  • Leadership Requirement: Strong, proven leadership experience required—ideally in a Medical Director or Lead Physician capacity. Candidates must demonstrate the ability to guide clinical teams and support operational oversight.
  • BLS certification (includes CPR).
  • Bilingual Spanish preferred.
  • Ability to meet productivity and clinical documentation standards.


Physical & Work Environment

  • Regularly required to sit, talk, hear, and use hands/fingers.
  • Occasionally required to stand, walk, reach, balance, stoop, kneel, or lift up to 10 lbs.
  • Exposure to contagious diseases (e.g., TB, AIDS, STDs) and potentially challenging patient interactions.
