JOB DESCRIPTION
Medical Records Coordinator
Position Summary
The Medical Records Coordinator is responsible for managing resident medical records in accordance with federal and state regulations, facility policies, and HIPAA requirements. This role works closely with nursing, therapy, social services, admissions, and administrative teams to ensure accurate and timely documentation throughout each resident's stay.
Essential Duties and Responsibilities
Maintain resident medical records, ensuring accuracy, completeness, and compliance with regulatory requirements
Process admissions, discharges, transfers, and record requests
Audit medical records for completeness and follow up with interdisciplinary team members regarding missing documentation
Manage electronic health record (EHR) systems and paper records as needed
Ensure all records are maintained in accordance with HIPAA and privacy regulations
Prepare records for surveys, audits, legal requests, and quality assurance activities
Coordinate release of information requests in compliance with applicable laws and facility policies
Maintain record retention and destruction schedules
Assist with scanning, indexing, filing, and archiving documents
Provide support and training to staff regarding documentation standards and medical record procedures
Collaborate with department leaders to improve documentation processes and maintain regulatory compliance
Qualifications
High school diploma or equivalent required; associate degree or coursework in health information management preferred
Previous experience in medical records, health information management, or long-term care preferred
Experience in a skilled nursing facility or healthcare setting strongly preferred
Knowledge of HIPAA, medical terminology, and healthcare documentation practices
Proficiency with electronic medical record systems and Microsoft Office applications
Strong organizational skills with exceptional attention to detail
Ability to prioritize tasks and manage multiple deadlines
Excellent communication and customer service skills
Preferred Qualifications
RHIT, RHIA, or other health information management certification preferred but not required
Experience with long-term care software such as PointClickCare or similar EHR platforms
Familiarity with Medicare, Medicaid, and long-term care regulations
