Juvenile Justice Services ProgramLocation: Nassau CountySchedule: Full-Time (35 hours/week) Non-Traditional Schedule We are also honored to announce that the Hauppauge Industrial Association of Long Island (HIA-LI) an advocate for its member organizations since 1978 has recognized HFY as their Not-For Profit Business Award honoree for Excellence in 2025!About the Program:The Juvenile Justice Services Program at Hope For Youth provides case management restorative justice conferencing and clinical services for up to 40 youth referred by the Nassau County Probation Department. These youth may include:Persons In Need of Supervision (PINS)Juvenile Delinquents (JD)Juvenile Offenders (JO)JDRaise the AgeAdolescent OffendersOur program focuses on family-centered treatment services that have proven effective in reducing delinquency risk and avoiding out-of-home placement.Position Overview:The Case Planner plays a critical role in supporting youth and families by improving individual functioning and family dynamics. This is a field-based position requiring weekly contact with youth and their families throughout Nassau County. The Case Planner also delivers direct services such as:Case managementLife coachingParent skills trainingCoordination with schools courts and other community partnersThe Case Planner reports directly to the Program Supervisor.Key Responsibilities:Maintain weekly contact with youth and families (including evenings/weekends) utilizing your own vehicleImplement all components of the program model for a caseload of up to 20 youthProvide case management life coaching and parent skill trainingMake collateral contacts with schools service providers and justice system representativesConduct CANS 2.0 assessments (initial and follow-up)Attend school meetings and court appearances as neededRegularly review and update service plansParticipate in transition and discharge planningComplete timely and accurate documentationAttend supervision treatment team meetings and ongoing trainingsTo Be Successful You Must Be Able To:Adhere to agency policies program procedures and ethical codes of conductSupport and empower families to maintain youth in the homeAchieve positive outcomes as outlined in NYS County and agency standardsBe flexible and responsive to the needs of youth families and the programComplete all tasks and responsibilities as required by Hope For Youth and regulatory agenciesMinimum Qualifications:BSW or Bachelors Degree in a related field plus 2 years of casework experienceOR MSW or Masters in a related fieldExcellent verbal written and computer skillsAbility to work a flexible non-traditional schedule (some evenings and weekends)Access to a reliable vehicle for regular home visits throughout Nassau CountyBilingual (Spanish/English) preferredPhysical Requirements:Ability to read/write reports and log entriesWalking/standing for prolonged periodsBending/stooping kneeling/squatting climbing stairsReaching overhead repetitive wrist/finger movementsRegular driving using personal vehicleWhy Join Hope For YouthAt Hope For Youth youll be part of a mission-driven team making a real impact in the lives of youth and families across Long Island.
