We also provide attractive working conditions and opportunities for career growth through professional development.Discover why Cooper University Health Care is the employer of choice in South Jersey.Short DescriptionNavigator Specialists in the Center for Healing provide ongoing supportive services to patients with substance use disorders.They provide education service coordination service linkage and support to patients within the healthcare social service and justice systems.Provide ongoing care-coordination and case management services.Guide linkage to appropriate clinical services including medication-assisted treatment (MAT) behavioral health treatment and social services to individuals with Substance Use Disorder.Provide education coordination navigation linkages and support to individuals with Substance Use DisorderFoster one-on-one supportive relationships built on trust and respect and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcomeProvide continuity of care by meeting patients in various clinical care settings including the following:Emergency department visits addiction treatment appointments medical appointments social service appointments and therapeutic appointments.Identify health and wellness needs barriers and strengths including physical health medical and addiction treatment social support occupational and intellectual strengths and goals financial and housing etc.Perform outreach by meeting with patients in the community and act as allies and advocates for patient servicesAttend Addiction Medicine clinical and programmatic conferences and meetings as requiredExperience RequiredHigh School Diploma REQUIRED1 year of relevant experience ifcandidatehasbachelorsdegree inrelatedfield or 2 years of relevant experience ifcandidatehasassociatesdegree or is a Certified Peer Recovery Specialist.4 years of relevant experience either clinical or lived ifcandidateis a Certified Medical Assistant.4 years of relevant experience either clinical orlived accepted in lieu of bachelorsassociatesor relevant certification.Lived experience with navigating the criminal-legal system preferred.Bilingualcandidatespreferred.Education RequirementsRegistered Nurse ORLicensed Practical Nurse with 2 years lived experience ORBaccalaureate (BA) degree with 2 years lived experience ORAssociate degree with 4 years of lived experience ORCertified Medical Assistant with 4 years of lived experienceSpecial RequirementsCommunication Ability to communicate with patients visitors and coworkersBi-Lingual preferredSalary Min ($)USD $24.00Salary Max ($)USD $40.00Required Experience:IC Key Skills Bidding,Apps,Benefits,Corporate Recruitment,Android Development Employment Type : Part-Time Experience: years Vacancy: 1
