Job Description

OverviewCatholic Charities is seeking a full-time Case Manager for our IRAP department (Immigration and Refugee Assistance Program) to help clients in Erie County at our Herkimer Office. This position is responsible for conducting health outreach for the Refugee Health Promotion program assisting with recruiting eligible individuals for wellness groups co- facilitating wellness groups and planning/executing health outreach events in collaboration with community partners. Health Outreach Case Manager will work directly with Medical Case Manager to coordinate services to promote health and mental health amongst newly arrived refugees and vulnerable populations.
