Job DescriptionJob DescriptionAbout the Role:The LPN plays a critical role in supporting the DON by ensuring the smooth operation of clinical and administrative tasks within a Homecare Office. This position requires PRN direct patient care, and some travel, which contributes to accurate and efficient medical care. The LPN also manages clerical duties such as scheduling appointments, organizing patient records, and performing data entry to maintain comprehensive and up-to-date documentation. By assisting in implementing solutions and coordinating with the DON, the LPN helps enhance patient experience and operational efficiency. Ultimately, this role is essential in bridging the gap between patient care and administrative support to facilitate high-quality home care delivery.Minimum Qualifications:Active LPN license in good standing in the state of practice.Proficiency in clerical and administrative tasks including scheduling, filing, and data entry.Pass a Criminal Background Check.Preferred Qualifications:Experience in administrative assistance within a healthcare/ home care setting.Familiarity with electronic health record (Well Sky) systems.Strong organizational skills with the ability to multitask effectively.Previous experience traveling to patient's locations for home care services.Responsibilities:Contact and assist patients, ensuring a welcoming and organized environment.Record and monitor vital signs such as blood pressure, temperature, and pulse accurately.Schedule patient appointments and manage calendars for DON and PRN nursing staff.Maintain and organize patient files, ensuring confidentiality and compliance with state regulations.Perform data entry tasks related to patient information, and medical records.Complete home visits when needed.Coordinate travel arrangements/ mileage for PRN nurses.Support the implementation of administrative and clinical solutions to improve workflow efficiency.Skills:The LPN utilizes clerical and administrative skills daily to manage patient scheduling, organize medical records, and perform accurate data entry, ensuring seamless office operations. Executive administrative assistance skills support efficient communication and coordination among home care staff, DON, and patients. Clinical skills, including taking vital signs and assisting with home visits, are essential for providing direct patient care, supporting the DON and nursing team. Organizational skills enable the LPN to maintain a structured environment, manage multiple tasks, and adapt to dynamic home care settings. Additionally, the ability to travel to patient's homes are required.
