We are seeking an experienced Hospice Clinical Manager to lead our hospice care team in Watertown. This team plays a vital role in providing meaningful support to terminally ill patients and their families ensuring comfort dignity and high-quality clinical care.Key ResponsibilitiesLead and mentor a team of Home Hospice Case Managers fostering a collaborative and supportive work environment.Oversee and coordinate patient care services ensuring seamless communication between all providers and disciplines.Manage hospice staff operations including recruitment hiring onboarding training performance development and ongoing support.Supervise patient assessments and ensure care plans align with hospice standards and patient needs.Collaborate with caregivers insurance agencies healthcare partners and community resources to ensure comprehensive care delivery.Educate patients families and team members on the benefits and philosophy of palliative and hospice care in the home setting.QualificationsBachelor of Science in Nursing (BSN) required.Active RN license.Minimum of 3 years of hospice or oncology nursing experience.At least 1 year of leadership supervisory or managerial experience.Position DetailsFull-time permanent roleMondayFriday daytime scheduleCompetitive compensation strong benefits and meaningful work with a mission-driven teamIf youre an empathetic experienced nurse leader looking to make a profound impact we encourage you to apply.
