One Care Plan Advocate at Old Colony Elder Services

OCES aims to support elders and individuals with disabilities by providing vital information and coordination of services that promote healthy safe lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities for as long as possible.To that end OCES is looking for a dedicated One Care Plan Advocate to work in our Senior Care Options (SCO) department to empower our consumers to meet their needs for Long Term Services and Supports (LTSS) through assessment person-centered care planning advocacy and participation in each consumers Interdisciplinary Care Team (ICT); promote individualized care plans that are cost-effective and improve quality of life.What youll do:Conduct comprehensive needs assessments and develop individualize care plansSupport and advocate for person-centered care planning and provision of LTSS in ways that promote independent livingRepresent the needs of the OCP enrollee by identifying communication and support needs advocating for the enrollee and providing education on LTSS to the ICT and enrolleeMonitor the provision and effectiveness of community services according to the ICPIdentify community-based alternatives to long-term institutional careWhat youll bring:Your passion for helping others and desire to make a differenceAdaptability to be able to change as the needs of consumers and/or programs changeAt least two years of experience in the human services fieldBachelors degree in Social Work or related field highly preferred; Current licensure if appropriateWhat youll get:Ability to work with an enthusiastic team of like-minded individualsOpportunity to develop professionally in your chosen careerGreat work/life balance: 35-hour work week with some flexibility; standard M-F workweek; hybrid work environment (home office and home visits).Exceptional benefits: health benefits available on day 1; generous paid time off policies; company paid LTD and life insurances 401K planAPPLY TODAY!OCES is an open welcoming organization that respects our diversity.
