While this position is primarily administrative there are times when you will meet with patients and families face-to-face and conduct telephonic outreach as part of the care coordination process.This is an excellent opportunity for someone who thrives in a collaborative fast-paced environment and is excited to help build the future of home-based care.About the RoleCollaborating with clinical nurses providers and the Business Operations Manager the Operations Coordinator supports the daily functions of Home Hospital. Your role includes assisting patients and families with screening consent and enrollment. You will monitor admissions and referrals manage supplies and equipment and help maintain critical workflows to deliver quality care at home.You will acquire a detailed understanding of the Home Hospital model and work independently onsite. You will also work alongside colleagues throughout the Mass General Brigham system.Job SummaryKey Responsibilities: Serve as a liaison across departments teams and external partnersCoordinate patient screening consent enrollment and transfer into the Home Hospital programProvide onsite operational support: manage supplies equipment scheduling and day-of admission logisticsAssist with arranging vendor services such as meal delivery infusion services or home health supportConduct face-to-face interactions and telephonic outreach with patients and families as neededTrack and report daily census and other operational metricsConduct safety and criteria screenings and assist with patient consentingSupport compliance with regulatory quality and patient safety standardsMaintain supply levels organize materials and build supply kits for admissionCollaborate with administrative managers across Home Hospital sitesAssist with onboarding and provide general administrative support as assignedQualificationsRequired: High school diploma or equivalentPreferred: Associates degreeExperience: 23 years in an administrative or operational support role ideally in healthcareKnowledge of hospital operations and regulatory requirements is a plusKnowledge of EPIC is a plusAdditional Job Details (if applicable)Why Join UsAs a foundational member of our Home Hospital operations team you will help build the future of home-based acute care.
