Job Description
We are seeking a compassionate and skilled Physician Assistant to join our multidisciplinary team in our outpatient diabetes clinic. This is an excellent opportunity to work with a collaborative, patient-centered group dedicated to delivering comprehensive, evidence-based care to individuals living with diabetes and related endocrine conditions. This position is based in Keene, NH, a beautiful college town in southwestern New Hampshire with outdoor recreational opportunities, cultural venues, and a vibrant downtown. Key Responsibilities:Conduct patient visits and develop and implement individualized care plans focused on disease prevention, glycemic control, and long-term healthCollaborate closely with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver holistic patient careEducate patients and families on diabetes management, lifestyle modifications, and medication useOrder and interpret labs, adjust medications, and monitor treatment outcomesParticipate in quality improvement initiatives and contribute to program growthQualifications:Physician Assistant with ability to obtain/maintain New Hampshire licensureNational certification as a PA, FNP, or AGNPPrior experience in endocrinology, diabetes care, primary care, or related specialtyStrong clinical judgment, communication, and patient education skillsWhy Join Us:Outpatient, weekday schedule with no callSupportive, collaborative team environmentOpportunities for continuing education and professional developmentCompetitive salary and comprehensive benefits packageAbout Cheshire Medical Center:Cheshire Medical Center, a member of the world-class Dartmouth Health system, advances the health and wellness of the communities throughout the Monadnock Region through a combination of outstanding care and exceptional service.
