To learn more visit.Role Scope:This is an opportunity for a mission critical Outreach & Enrollment Supervisoris responsible forclosely developing and managing a team of Habitat Health Outreach Specialists Enrollment Specialists and Intake Coordinator as well as key community partners. Critical job responsibilities will include ensuring prospective participant enrollment success managing the teams individual and overall performance against targets on a monthly and yearly basis and supporting the sourcing andmaintainingof local outreach channels for prospective participants. This supervisor willbe responsible forleading new participant growth efforts for the Sacramento Center location including ensuringfuture prospectshave a comprehensive financial and functional assessment and meet all compliance requirements by DHCS and CMS to enroll in our PACE program.The Outreach & Enrollment Supervisor will also be part of the Sacramento Centers leadership team and work cross-functionally to ensure that new participants thrive at our centers and Habitat Health continues to bea great placeto work for our team members. The Outreach & Enrollment Supervisor is expected to review metrics daily support growth initiatives coach staff members and ensure the Outreach and Enrollment teamoperatessmoothly and successfully in collaboration with the Sacramento Centers Executive Director and Habitat Healths Growth leadership.Core Responsibilities & Expectations for the RoleManage and develop a team of 5-10 quota-carrying Outreach and Enrollment Specialists as well as Intake Coordinators across 1-2 PACE centersCarry and deliver against targets for monthly team contribution to participant growthDevelop both the long-term vision and strategy for the team while managing day-to-day Outreach & Enrollment team operationsProvide an unmatched experience for participants and prospectsTroubleshoot bottlenecks and proactively remove operational obstaclesProvidereal-time guidance on complex cases eligibility considerations family dynamics and barriers to enrollmentDevelop and implement strategies toidentifyand strengthen referral pathways with community organizations providers and partners in the greater Sacramento areaSupport Outreach Specialists and the Growth leadership team on local marketing events to engage key stakeholders in the greater Sacramento areaBe an ambassador in the local community to help engage prospects building relationships with other healthcare providers social / community organizations and marketing partnersIdentifyand share best practices across the organizationProvide skill training mentoring and quality checks on participant and prospect engagementConduct auditing efforts to ensure quality assurance and to meet DHCS and CMS guidelinesRecruitretain and nurture team of Outreach & Enrollment Specialists with high potential andstrong performancehistoryServe as a cross-functional leader by collaborating across the organization including supporting key strategic initiatives related to center and clinical operations participant experience and journey and health plan operations as they intersect with the Outreach & Enrollment teamRepresent the Sacramento Outreach & Enrollment team at both the Growth leadership and Sacramento Center leadership levelsfacilitatingcross-functional engagement to align on and drive progress against key initiativesQualifications:Minimum of 3 years in healthcare sales outreach intake enrollment social services healthcare navigation case management or community engagementA minimum of one year experience working with the frail or elderlyStrong computer skillsAbility to create and analyze excel spreadsheetsExperience managing through a CRM systemExperienceutilizingand navigating EMR systemsAnalytical approach toward sales data & performanceComfort with personal sales quota in addition to management responsibilitiesExcellent relationship and people management skills with the ability to motivate a teamStrong focus on data and analytics to drive targeted activities and return on growth effortsAbility to manage competing priorities whilemaintaininga positive attitudeInclination to work in a results-oriented roleExperience working cross-functionally to streamline workflowsAbility to navigate complex situations with families caregivers and external partnersWilling to work weekends as necessary particularly for community eventsProof of medical clearance for communicable diseases including a TB testProof of all immunizations is up to dateProof of valid CA drivers license personal transportationgood drivingrecordand auto insurance as required by State lawNice to have:Prior experience in a sales management role or other comparable management positionFamiliarity with the PACE program or other elderly care organizationsBilingual: Spanish/Mandarin/Cantonese preferred.Deep understanding of the local Sacramento health care communityCompensation:Wetake into accountan individuals qualifications skillset and experience in determiningfinalsalary.
