Job Description

We are seeking a dedicated Registered Nurse (RN) or Physician Assistant (PA) to join our Population Health Division. The ideal candidate will have experience or strong interest in value-based medicine high-risk patient management and addressing gaps in care. This role plays a key part in improving health outcomes for marginalized and underserved populations supporting our mission to deliver equitable high-quality care.Key ResponsibilitiesMonitor track and close gaps in care across value-based programs.Conduct outreach and follow-up for high-risk patients to support care compliance.Review medical records and clinical data to identify required preventive screenings chronic disease management needs and overdue health measures.Collaborate with providers care coordinators and the quality team to improve population health metrics and clinical performance outcomes.Support patient education on chronic disease preventive care and care plan adherence.Assist with the development and implementation of value-based care initiatives including workflows that improve quality scores and patient engagement.Participate in quality audits documentation reviews and reporting for payor-based value programs.Promote health equity by helping remove barriers to care for underserved patient groups.QualificationsActive RN or PA license in New Jersey (required).Experience with value-based medicine quality measures or population health strongly preferred.Knowledge of HEDIS measures preventive screenings chronic disease management or care coordination is a plus.Strong communication assessment and patient engagement skills.Ability to work independently manage follow-ups and prioritize patient needs.A passion for improving health outcomes in marginalized communities.What Were Looking ForA mission-driven clinician who is:Detail-oriented and proactiveComfortable reviewing data and navigating EMR systemsCompassionate and committed to patient advocacyMotivated to support organizational goals in value-based careBenefits:401(k)401(k) matchingDental InsuranceHealth InsurancePaid sick timePaid time offVision InsuranceMedical Specialty:Primary CarePhysical Setting:Private practiceExperience:Nursing: 2 years (Preferred)Language:Spanish (Required)License/Certification:BLS Certification (Preferred)RN (Preferred)Ability to Commute:Essex County NJ (Required)Ability to Relocate:Essex County NJ: Relocate before starting work (Required)Work Location: In person Key Skills Bidding,Apps,Benefits,Corporate Recruitment,Android Development Employment Type : Part-Time Experience: years Vacancy: 1
