This is a casual/PRN position. Casual/PRN employees work on an as needed basis to meet the needs of the business. Work days/hours will often vary from week to week. The role is to oversee and ensure the appropriate and safe utilization of medications for patients within the facility. The pharmacist must maintain competency in unique aspects of drug therapy for the patients for whom they provide care including factors related to patients of all age groups including neonates pediatrics adults and geriatrics.Monitors drug therapy to evaluate appropriateness of use drug dosage form regimen route therapeutic duplication and drug interactions. Pharmaceutical care provided by assessing patient demographics; reviewing patient medication histories; reviewing patient medication profiles; establishing desired outcomes; developing therapeutic care plans; determining alternatives for achieving outcomes; educating patients and or family on drug therapy; monitoring patient outcomes; and monitoring potential adverse drug-drug and drug-food interactions. Mastery of the use of Cerner pharmacy and hospital information system application is required. As well as proficiency with all technology associated with the medication management process to include but not limited to Automated Dispensing Cabinets Automated TPN compounder and Automated dispensing equipment such as packaging machines/carousels. Understanding and compliance with intravenous compounding policies and procedures is required. Responsible for adhering to policies procedures staffing safety metrics and financial metrics. Education to healthcare professionals and patients is a requirement.Must maintain compliance with all state and federal regulations regarding medications.The role is to oversee and ensure the appropriate and safe utilization of drugs of hospitalized or emergency department patients for the day/evening and weekend shifts.Responsibilities:Essential Job Functions1. Patient Care ServicesReview medication orders for appropriate dose duration frequency dosage form indication allergy interactions and contraindications. Ensure that medication regimen is consistent with all organizational medication use guidelines and policies or other evidence-based guidelines in the absence of organizational guidelines which includes (but is not limited to): antimicrobial use intravenous to oral transition renal dosing anticoagulation and pharmacokinetics/serum drug concentration protocols.Adjusts medication regimen based on approved protocols and/or consultation with interdisciplinary medication therapy for potential or actual drug interactions (e.g. drug-drug drug-disease drug-allergy food-drug or contraindications) and outcomes.Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered careParticipates in interdisciplinary patient care roundsand oversees pharmacy resident and student participation in patient care rounds when applicable.Performs and/or reviews medication histories comparing prescribers orders to the patients active medication list(s) and works with prescribers to make changes as necessaryDocument activities related to medication reconciliation in medical record across patients transition of care (including but not limited to: admission transfer and discharge)Complete discharge medication review facilitate the discharge process including discharge prescriptions and/or provide discharge education per department procedures or upon request by patient or health care provider. Ensure compliance with health system inpatient drug formularyCompletes medication profile review when staffing clinical area reviewing the following factors as appropriate:Age sex health weight renal & hepatic function or other physiological parametersPregnancy and lactation statusAppropriateness of the dose route rate timing & frequency of administration and indicationReview for therapeutic duplicationAvailable laboratory values and microbiology resultsAppropriateness of antimicrobial therapy (including anticipated/planned stop dates)Potential drug interactions and contraindications2. Coordination of Distribution SystemsCoordinate and participate in the preparation and distribution of medications including communication of pending issues/transactions with team members and other health care professionalsCoordinate drug delivery and documentation as appropriate.Complete and pass competencies associated with sterile non-sterile and hazardous drug compounding management as assigned based on area of workUnderstand safety precautions including hazardous materials handling and proper precautions against blood borne pathogens as it relates to area of practiceMonitor that the medication use system is providing the right patient the right drug the right dose the right route and the right time with the right outcomes.Serve as a liaison for inpatient patient information for outlying patient needs (retail pharmacies home health physician clinics etc.)3. Quality ImprovementIdentify and report adverse drug events (adverse drug reactions and medication errors) in the organizational reporting systemAssure compliance with health system hospital and departmental policies and proceduresMonitor compliance with state and federal laws and regulations related to the practice of pharmacy as well as standards and regulations for other regulatory agenciesParticipate in departmental and hospital committees as assigned4. Training Education and Community ServiceProvide direct supervision of certified pharmacy technicians pharmacist interns and pharmacy students and assist with their education training and developmentProvide drug information/education to health care providers and patients upon request and as per department proceduresProvide in-services and lectures to health care team members as assignedPrecept and evaluate pharmacy residents and PharmD students on residency/clerkship rotationsProvide drug information and disease prevention programs to consumer and patient groups as neededFollows and understands the mission vision core values and Employee Standards of BehaviorComplete required departmental education/competency modules (Age Friendly etc.).5.
