Join us!We are currently seeking an experienced:MEMBER NAVIGATORJOB SUMMARY:Assists with access to care for CSNP members in collaboration with an RN case manager and nurse practitioner.Obtains information for members based on the model of care requirements.Obtains information to assist with evaluation of member needs.Communicate effectively with individuals across the care continuum.Documents member information in the electronic medical record.Reviews appointments and communicates pertinent information to the RN Case Manager(s) and nurse practitioner(s) for additional interventions.Collaborate with IDT for coordination of careCommunicate with members and/or families obtaining and providing information regarding care and needs.Strong communication skills and focus on customer needs and services.Assists with any member emergency as needed.Recommends and/or conducts specific areas of in-service or continuing education with patients and caregivers as appropriate.Assists with compliance of quality measures for CSNP programAssist with HEDIS measures and collection of dataAssist with collection and presentation of QA data for the CSNP programParticipates in ICT meetings for CSNP membersAssist with Medication AdherencePerforms other duties as assigned.JOB REQUIREMENTS:Must be a high school graduate, or equivalentMust be able to demonstrate computer skills, including use of e-mail, Microsoft Office Word and Excel.Must be able to demonstrate effective written communication skills, including but not limited to, writing reports and correspondence.Must be able to demonstrate effective verbal communication skills, including, but not limited to, speaking effectively to members, providers, families and staff and listening to members, providers and familiesDemonstrated ability to meet deadlines, and use time effectively and efficiently.Demonstrated successful experience working in an environment of change.
