OverviewPresbyterian is currently hiring a Peer Recovery Support Specialist in Clovis NM. This role focuses on providing Recovery Coaching to individuals facing mental health and substance use challenges.As a Peer Recovery Support Specialist youll help patients identify personal and environmental barriers to recovery connect them with the recovery community and serve as a mentor in managing both personal and family recovery. While not acting as a sponsor youll model resilience coping strategies and a commitment to ongoing recovery.Collaboration is key in this roleyoull work closely with the patients treatment team and when needed engage with their family and community to support a holistic recovery journey.This is a Full Time (1.0 FTE) position- Exempt: NoJob is based at Plains Regional Medical Center Clovis NMWork hours: DaysQualificationsEducation: High School Diploma/GED requiredKnowledge of community resources required.Register for Peer Certification class within 6 months after hireCurrent NM drivers license is preferredCertified Narcan educator preferredTrained in Motivational Interviewing Skills preferredPrevious experience working in hospital or ED setting preferredExperience working with confidential medical records and documentation preferred.Effective written and verbal communication skillsComputer skills familiarity with Microsoft Word Excel and Power PointFamiliar with readiness assessment tools and SBIRT.ResponsibilitiesCoordinates and communicates with hospital staff funders and community partners as required.Works independently and with a multidisciplinary team to provide services and deliverables as required by PHS and/or funders.Engages patient toward recovery using Motivational Interviewing Skills.Provides patient assessment(s).Provides Naloxone education in partnership with nursing and medical personnel.Identifies and provides community resources.Works with ancillary/community providers to ensure seamless transition for continuity of care.Supports patient in linkage with community appointment prior to discharge.Regularly contacts patient after discharge.Manages and maintains patient tracking and outcomes.Complies with hospital funding State and Federal regulations.Support Patients first and foremost uses their personal experiences to develop meaningful and trusting relationships with patients acting as a mentor. As someone who successfully managed their own recovery the Peer Recovery Support Specialist provides patients an example of what they can strive for in their recovery.Coordinate Patient Logistics in order to support the facility handles some logistics of patient arrival and departure; includes arranging transportation for the patient managing follow-up appointments and working with facility administrators and discharge staff members.Build Community works to build a community of both patients and staff that is supportive and conducive to patient growth and recovery. May assist in the creation and implementation of social activities and help create a positive environment for everyone.Identify Resources for patients; link up with various community organizations and resources to provide additional support for patients particularly as they transition from recovery back into the general community.
