Remote/work-from-home positions are not available unless specifically noted.Full Time Physical Therapist Case ManagerAnticipated Territory: Pacifica Half Moon Bay and La HondaIn partnership with other clinicians on the case you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively in their home environmentYour goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may may also supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans comprehensive health benefits and 401k (Up to 4% match). As a practice the majority of PT Case Managers are hired as PTIs. Once it is determined that an PTI can supervise 2 PTAs take on a larger case load and assume a higher level of decision making and autonomy the individual will be promoted to an PTII with a minimum salary of 135000 plus 20K bonus (Paid Quarterly).*You will be assigned a primary location; however you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies leaves of absence higher patient census or other client care needs.
