Locations: Livermore & El Sobrante, CA | $31.00 – $46.50/hour

Schedule: Full-Time | Monday–Friday, 8am–5pm

Department: Geriatric Rehabilitation | PACE Program

Work alongside physicians, nurses, social workers, and rehabilitation professionals to support seniors in maintaining mobility, dignity, and quality of life.


The Role

Under the supervision of a Physical Therapist, you will:

  • Implement individualized treatment plans focused on safety, independence, and functional mobility
  • Provide therapeutic interventions including exercise, gait training, neuromuscular re-education, manual techniques, and physical modalities
  • Educate participants and caregivers on assistive devices, mobility strategies, and adaptive equipment
  • Lead and assist with group therapy sessions
  • Collaborate with an interdisciplinary care team to ensure coordinated, high-quality care
  • Document services accurately and maintain therapy equipment and supplies
  • Travel locally to participant homes and multiple program sites as needed


Qualifications

  • Associate’s degree from an accredited Physical Therapist Assistant program
  • Current California PTA license (required)
  • CPR and First Aid certification
  • Valid California driver’s license, current vehicle insurance, and reliable transportation
  • Experience with frail elderly or geriatric populations preferred
  • Strong communication skills and collaborative approach
