Through a membership-based approach, patients are supported by an integrated care team—including a physician, dietitian, physical therapist, and care navigator working together to help patients fully bloom. We combine clinical care with personalized exercise plans and nutrition coaching, using technology and continuous support to help people make lasting changes to how they live, move, and eat.

The Opportunity

We’re looking for an ambitious licensed Physical Therapist to join Bloom360 as our Lead PT. This is a telehealth role where you’ll lead our movement program, working directly with members on overall fitness, injury prevention, chronic pain, and rehabilitation. You'll collaborate closely with our physician and dietitian and help shape how movement counseling gets delivered at Bloom360.

What You'll Do

• Conduct virtual functional movement assessments to evaluate mobility, strength, pain, and functional limitations within the context of each patient’s medical history
• Design and progress individualized home exercise programs (HEPs) that are evidence-based and aligned with patient goals and clinical conditions
• Deliver telehealth physical therapy across full scope, including therapeutic exercise, functional training, and pain management
• Collaborate closely with physicians and care team to integrate movement into overall care
• Support patients with musculoskeletal conditions, chronic pain, post-surgical recovery, and mobility limitations using modern pain science and functional rehabilitation principles
• Apply behavior change strategies to help patients build sustainable movement habits and overcome barriers (e.g., pain avoidance, time constraints, attitude)
• Educate patients on injury prevention, ergonomics, and safe, effective movement in daily life
• Track progress using functional and patient-reported outcomes, adjusting care plans accordingly
• Contribute to development of Bloom360’s movement care model, resources, and protocols
• Maintain accurate, timely documentation within the platform

What We're Looking For

Required Qualifications:

• Doctor of Physical Therapy (DPT) or licensed Physical Therapist (PT)
• Tech savvy with strong preference for demonstrated telehealth PT experience
• Strong understanding of functional movement screening, exercise prescription, and progressive programming
• Clear, confident communicator who can coach and educate members effectively over video
• Michigan licensure (DPT/PT)

Preferred Qualifications:

• Experience treating chronic pain, musculoskeletal conditions, or post-surgical rehab via telehealth
• Familiarity with prescribing exercise as medicine for metabolic and cardiovascular conditions
• Experience building video-based exercise libraries or member home program resources
