Employment Type:Full timeShift:Day ShiftDescription:Full-Time - 40 hours weekly; Days - 8 hour shift; New Hampton Medical CenterThis individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for clients home/residence.ESSENTIAL FUNCTIONS Evaluates clients to obtain and interpret data necessary for treatment planning and implementation in the clients home setting.Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.Develops an individualized plan of care involving the client specific to the needs identified while collaborating with the physician.Communicates with physician any requests to revise the plan of care.Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.POSITION QUALIFICATIONSGraduate of a university approved by American Physical Therapy AssociationLicensed in the state of Iowa to practice physical therapyMinimum of 1-year experience preferredCommunity or home health experience desirableOur Commitment Rooted in our Mission and Core Values we honor the dignity of every person and recognize the unique perspectives experiences and talents each colleague brings.
