DescriptionPosition at Vassar Brothers Medical CenterPurpose:Following a medically prescribed physical therapy program evaluates treats and educates pediatric adolescent adult or geriatric patients who have disabilities traumatic injuries pain disease or any dysfunction and helps them reach their maximum performance and assume a place in their community while learning to live within the limits of his/her capabilities. Demonstrates knowledge and skills necessary to provide appropriate care to the age of the patient served in the inpatient and/or outpatient setting.Essential Responsibilities1. Provides direct patient care services to ensure the effectiveness of physical therapy programs while adhering to safety policies and procedures.2. Evaluates patients and establishes an effective treatment plan to achieve set goals.3. Provides physician prescribed physical therapy treatment to all patients within the assigned caseload to return the patient to optimum and productive levels within the patients capabilities.4. Prioritizes caseloads and available treatment times to provide for maximum efficiency.5. Improves or minimizes residual physical disabilities of the patient by utilizing correct modalities of treatment and suggests variations.6. Recognizes contraindications to continuing with scheduled treatment and consuls with responsible Physician.7. Involves family or caregivers in treatment plan and discharge planning.8. Interprets the appropriate information needed to identify each patients requirements relative to age-specific needs.9. Performs appropriate documentation to effectively communicate the patients functional status to Physicians insurance companies the patient their caregivers.10.
