Our team is growing and looking for a dedicated:Physical Therapist(PT)Shifts Available:Full-Time &Part TimeService Areas with Openings: Southeast (Pearland)Responsibilities:Facilitate the rehabilitation of patients with emotional physical or mental disabilities.Implement physical therapy care plans.Coordinate care with the home healthcare team.Educate patients and family members about health conditions.Prepare timely complete and accurate clinical notes.Communicate effectively with patients caregivers healthcare providers office support and other care team members.Qualifications:Physical Therapist in the state of Texas.Home health experience highly preferred.Knowledge of home health regulations and general healthcare compliance.Experience with HCHB EMR.
