Physical Therapist (PT) – Home Health
Full-Time | up to $150,000 annually

We are currently seeking a licensed Physical Therapist (PT) to join our interdisciplinary home health team. This role offers professional autonomy, strong clinical support, and the opportunity to make a meaningful impact on patients’ mobility, independence, and quality of life.
Position Overview
As a Physical Therapist with Qualicare Home Health, you will provide skilled in-home physical therapy services, conduct comprehensive evaluations, develop individualized treatment plans, and collaborate closely with caregivers and clinical team members to deliver coordinated, outcomes-focused care.
Key Responsibilities
- Conduct initial evaluations, re-evaluations, and discharge assessments in accordance with Medicare and agency standards
- Develop and implement individualized treatment plans aligned with patient goals and physician orders
- Provide therapeutic interventions including gait training, balance activities, strengthening, range-of-motion exercises, and fall prevention
- Educate patients and caregivers on safety, home exercise programs, and proper use of assistive devices
- Collaborate with PTAs, RNs, OTs, and other interdisciplinary team members to ensure continuity and quality of care
- Accurately complete clinical documentation using OASIS and the agency’s EMR system in a timely manner
- Monitor patient progress, identify changes in condition, and communicate effectively with the care team
- Supervise Physical Therapy Assistants (PTAs) and aides in compliance with state and federal regulations
Qualifications
- Active and unrestricted California Physical Therapist (PT) license
- Minimum 1 year of home health experience, including OASIS documentation (required)
- Experience with EMR systems; WellSky/Kinnser preferred
- Strong clinical judgment, organizational, and communication skills
- Ability to work independently while collaborating with an interdisciplinary team
- Valid California driver’s license and reliable transportation
