This full-time, on-site position offers a rewarding opportunity to provide comprehensive, patient-centered medical care to older adults enrolled in a Program of All-Inclusive Care for the Elderly (PACE) model.Reporting to the Medical Director, the Primary Care Physician plays an essential role in the ongoing assessment, diagnosis, and treatment of participants, while collaborating with the interdisciplinary team to ensure coordinated, holistic care. The ideal candidate is passionate about geriatric medicine and thrives in a collaborative environment where compassion, respect, and innovation drive every interaction.Responsibilities Conduct initial, semi-annual, and annual assessments of participants in collaboration with the Interdisciplinary Team (IDT) Develop, implement, and evaluate individualized care plans for participants Provide primary and preventive medical care for participants in the clinic, home, or care facility Manage acute and chronic conditions with a focus on functional improvement and quality of life Supervise and collaborate with nurse practitioners and physician assistants as required Coordinate specialty consultations, hospital care, and nursing facility admissions as appropriate Participate in IDT meetings, daily case reviews, and quality improvement initiatives Serve as a resource for geriatric best practices and chronic disease management Complete accurate and timely documentation of all care and communications Support a positive and collaborative team culture that emphasizes dignity and independence for participantsWhat Are the Benefits?
