Plan of Care Specialist ProgressiveHealth of Indiana LLC Jasper INWe are looking for an energetic and customer service-driven Plan of Care Specialist to join our team! This position will maintain therapy Plans of Care and support front-office operations within our patient settings. This is a full-time in-office position with a weekday day-shift schedule. No previous experience is needed but would be ideal to have. This is also a wonderful opportunity to expand your knowledge and gain experience in the healthcare industry. Come be a part of a growing organization in a progressive supportive and innovative team!Monday - Thursday 7:30AM - 4:30PM & Friday 7:30AM - 4PMWhat will you be doing in this roleMonitoring and managing Outstanding Plan of Care (POC) Approval Reports and related POC work queues with accuracy and timelinessUploading and maintaining precise POC documentation within required timeframes with accurate approval datesCoordinating with provider offices to ensure timely POC certification and properly documenting all communicationCollaborating with clinic partners to reduce delays eliminate workflow barriers and uphold documentation qualityMonitoring aging POCs and securing necessary Delayed POC certifications to ensure compliance standards are metScheduling and recapturing appointments missing in action patients and pro-actively rescheduling appointmentsCoordinating with the patient and/or referring physician to obtain referralsDocumenting referrals as directed in appropriate logsScheduling initial evaluations and follow-up visits to yield optimal visits per treatment planVerifying patients insurance in advance to ensure that necessary prior authorizations by payors have been obtainedEducating patients confidently about their coverage and payment responsibilities as it relates to therapy servicesEnsuring patients understand and sign necessary documentation related to insurance coverage for services deemed non-coveredCollecting all monies that are due prior to each visitCould include co-payments deductibles co-insurance and payments on statementsAny other duties relating to the operations of the clinic that might be assigned by the Site Coordinator Revenue Cycle Department or Patient Experience CoordinatorThis does not necessarily list all responsibilities duties requirements or efforts associated with the job.
