The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.Position Details:Mercy Clinic 65 Prime Plus - North County 637 DunnSchedule: Monday - Friday, 0800-1630No weekends or holidays! Mercy 65 Prime+ Clinics are a new primary care service designed specifically for Medicare beneficiaries. Patients receive extended office visit times and closer monitoring of chronic conditions like high blood pressure, diabetes, and heart disease.
