Three (3) unsuccessful attempts at licensure will be permittedHow you will make a difference (job overview):Therapist direct service training position under the direct supervision of the Director of Treatment Services who assists in preparing Treatment plans writing weekly progress summaries discharge summaries Quarterly reports HS 231s and any court reports general casework services psychotherapy and therapeutic groups/activities to assigned residents and their families. Coordinates treatment within SVAC and with other agencies such as Child Protective Services Juvenile Probation Department and County Department of Mental Health other activities include interdepartmental consultation. Prepares a comprehensive written psychosocial evaluation and assessment on assigned adolescents in addition to an intake note regular progress notes weekly summary notes and Day Treatment Intensive daily notes. Responsible for permanency planning including assessing residents readiness for discharge and coordination planning with and lead treatment team; making appropriate referrals for placement case management and aftercare services; coordinates benefits with Business Office personnel and maintains knowledge of community resources. Communicates with residents their legal status and rights under applicable law and acknowledges in writing their understanding of same. Provides consultation as needed with families and the Case Manager on issues regarding admission discharge and aftercare plans. Leads or co-leads psychotherapy psycho-educational and therapeutic activity groups.
