Job DescriptionJob DescriptionBenefits:Dental insuranceHealth insurancePaid time offVision insuranceDescription:The Program Director for Sponsored Residential and Community Engagement services will oversee all staff, Sponsors, and direct care issues and services, ensuring effective leadership-based management of the program with an emphasis on high-quality care delivered in an effective manner by performing the duties outlined below.Duties and Responsibilities: Conducting Quality Assurance with the client; Managing and Oversee client recruitment Staffing/ Referral review for Sponsored Homes, Community Engagement; Case Management; Initials Admission Process Supervise all, including scheduling, client crises, and leadership interventions, as needed Monitors and ensures interventions are performed face-to-face outside the facility, and that each person supported receives required number of contacts per service definition Plans meeting agenda, prepares supporting materials, and facilitates meetings to include onboarding and interest meeting trainings Communicates effectively in all areas of practice, including how to motivate, and engage colleagues around a common goal Ensures that clinical processes, policies, and procedures are adhered to. Serve as a resource for SRS staff and Sponsors to guide to appropriate resources Ensure all structured and scheduled services commence at the designated timeframes. Other duties as assigned Positively promote and communicate information about ID/DD program to individuals, families, community members, other providers, CSBs, DSS workers, and other agencies. Develop, conduct, and approve assessments for new and current Individuals. Develop and ensure on-going implementation of Individual Support Plans, Behavioral and Therapeutic Treatment Plans. Review and interpret policies and regulations to ensure adherence to DBHDS, Human Rights, DMAS, and/or other State, Federal, or Local regulations. Complete appropriate serious incident reports in CHRIS system as needed. Ensure that all Level II and Level III serious incidents are entered into CHRIS within 24 hours of being notified. Establish appropriate structures and processes by using best practice tools and resources, such as Root Cause Analysis, the 5 Whys, etc. in relationship to identifying cause and effects and mitigating serious incidents and Infection Control concerns and issues. Coordinate the development and review of relevant/required risk and serious incident management policies, procedures, and guidelines. Make recommendations to reduce, prevent, or eliminate risks identified. Create and maintain risk audit binders and database and review with risk management team, leadership, and staff in relation to issues arising. Facilitate and participate in health and safety training as required and maintaining records of all health and safety training. Promote and maintain a safe working environment for all staff and ensure that all Risk Management & Serious Incidents, and Health & Safety Strategies, policies, and procedures are implemented. Conduct meetings and orientations with potential sponsors. Support with completion of initial paperwork and necessary trainings for new sponsors. Create and maintain current and accurate files for Individuals participating in Sponsored Residential program. Attend plan meetings for current Individuals and intake meetings for new sponsors and ensure the completion of necessary documents and receipt of insurance, financial, and other needed verifying documents. Advocate for Individuals and staff as needed. Coordinate and conduct monthly supervision meetings and complete meeting minutes. Provide input and recommendations (verbal and/or written) regarding issues of potential Individuals and sponsors, admissions, and discharges. Assist in maintaining sponsor training folders and provide ongoing support to sponsors as needed. Collect and review monthly documentation (i.e. progress notes) from providers. Conduct inspections of sponsored residential homes and provide recommendations and information of deficiency to support in gaining and maintaining compliance of homes and property. Review support data and progress notes in order to modify/revise individual support plans, behavioral supports, treatment plans, etc. as needed. Accurately enter case information into WaMS electronic system. Monitor Individuals eligibility of Medicaid and waivers and support with maintaining and reinstating benefits as needed. Maintain valid Virginia drivers license and auto insurance. Complete and maintain necessary trainings as identified by DBHDS, DMAS, HOU, and other state and/or federal agencies. Adhere to ethical and HIPPA regulationsQualifications: The Program Director will have a Bachelor's degree with at least 5 years of related experience OR 3 years of documented experience within the mental health field. The Program Director must have efficient written and verbal communication skills, problem-solving skills, demonstrate good decision-making skills, be able to multitask, to respond to crisis/ emergency events, and be able to manage multiple responsibilities. They must be energetic, innovative, outgoing, have a strong and assertive personality. Applicant must have two years of experience with a display of direct care in the ID/DD population. (Preferred) Knowledge of person-centered documentation, preferred knowledge of WaMS and Delta (CHRIS) systems, direct support experience/experience with ID/DD population, knowledge of case management.Licensure: Complete a criminal background check/ CPS Central Registry check Tuberculosis screening Valid state drivers license and current auto insurance CPR/First Aid Training Certification and Crisis Wave training (within 15 days of employment)
