Job Title: Behavioral Health Social Worker (Psychiatric Outpatient Services)Location: Georgetown, DE 19947Duration: Max 1yearwork Mode: On-siteShift : 1stJob SummaryThe Behavioral Health Social Worker performs professional social work in a psychiatric outpatient setting, providing support to individuals with developmental disabilities, mental illness, or emotional disturbances. The role focuses on helping clients achieve social, emotional, economic, and physical stability along with the highest possible level of personal independence. This position ensures that services are delivered in alignment with accepted professional standards and best practices.Key ResponsibilitiesClinical & Case Management ServicesConduct comprehensive assessments to identify client needs, strengths, and areas of risk.Develop and implement individualized, person-centered treatment plans.Provide direct behavioral health services, including counseling, crisis intervention, and ongoing support.Monitor, manage, and document client risk, safety concerns, and progress.Support clients in achieving emotional and social readjustment within community settings.Coordination & CollaborationCoordinate care across multiple service providers to ensure continuity of support.Refer individuals to appropriate community and social service agencies based on identified needs.Provide consultative support and guidance to state agencies, private organizations, and community partners.Administrative & Compliance DutiesMaintain accurate and timely documentation in compliance with Department of Health and Human Services (DHHS) standards.Ensure services support the mission and integrity of the DHHS Division of Mental Health and Substance Abuse.Participate in multidisciplinary team meetings, case reviews, and professional development activities.Reporting StructureReports directly to an administrative superior.Works independently while contributing to the goals and standards of the department.Required QualificationsBachelors or Masters degree in Social Work or a related behavioral health field (as required by state).Experience working with individuals with mental illness, developmental disabilities, or emotional disorders.Knowledge of behavioral health case management principles and person-centered practices.Key Skills & CompetenciesStrong clinical assessment and treatment planning abilities.Ability to manage crisis situations with calmness and professionalism.Excellent communication, documentation, and interpersonal skills.Ability to collaborate effectively with multidisciplinary teams and external agencies.Understanding of community resources and social service systems.
