This position offers an opportunity to make a meaningful difference in the lives of residents while working within a collaborative and growth-oriented organization.Key Responsibilities:- Lead and coordinate the Individual Support Planning (ISP) process for residents in Shores Residential homes.- Oversee the development implementation and monitoring of active treatment plans and program services.- Ensure all services comply with state and federal regulations and organizational policies.- Serve as a case manager advocate and liaison for residents families and service providers.- Participate in a rotating on-call schedule to provide 24/7 shift support for residential homes.- Collaborate with interdisciplinary teams to assess resident needs and adjust care plans accordingly.- Document all services interventions and progress accurately and timely.- Provide guidance and supervision to direct care staff regarding treatment plans and regulatory compliance.Skills and Qualifications:- Bachelors degree in Human Services Psychology Social Work or a related field.- Valid certification as a Qualified Intellectual Disability Professional (QIDP) or eligible for certification.- Prior experience working with individuals with intellectual and developmental disabilities preferred.- Strong knowledge of state and federal regulations related to residential services.- Excellent communication advocacy and interpersonal skills.- Ability to work independently and as part of a team.- Flexibility to work occasional evenings and weekends as needed.- Valid drivers license and reliable transportation may be required.We foster a supportive and inclusive work environment that values professional development and growth.
