Job DescriptionJob DescriptionSalary: $20-$26/hourPosition SummaryThe Qualified Mental Health Professional is responsible for providing individual skills training, mentoring, casemanagement and care coordination services for clients in the Intensive In-Home Program. These services areprimarily provided in the home.Essential Duties and ResponsibilitiesManages a caseload of clients, providing services on an intensive basis and utilizing short-term, solutionfocused treatment strategiesClearly identifies problem areas and needs, strategies, and service objectives supports in determining andmeeting service objectives and to identify additional resources and supportsPrepares written service plan, incorporating input from clients, involved service providers and wraparoundteam members, and clearly identifying problemProvides psycho-educational service to clients, including individual, family, and interventions andissues-specific or educational groups (parenting skills, anger management)Responds to crisis situations with twenty-four hour a day availabilityDocuments case activities for both clinical and billing purposesPrepares progress reports as required by referral source or service purchaserParticipates in Child Specific Team, Family Assessment and Planning Team, and other interdisciplinary,diagnostic, or planning meetingsCollaborates and coordinates with other case involved service providers and professionals, such as attorneys,school personnel, social workers, and probation counselorsLinks clients with external programs or services, such as health services, recreational activities, childcareservices, financial resources, employment resources, AA/NA groups, transportation resources, and othersAttends court hearings when families are court-involved and presents testimony and treatmentrecommendations as requested by attorneys, referral source, the court, or clientGuides, collaborates, and coordinates with follow up services with various community resources to includementoring services and case management services, and ensuring the adequate exchange of information isprovidedMeets every week with Clinical Director to provide clinical guidance, assess dynamics and needs of eachcase, determine the appropriateness of therapeutic strategies, and assess progress of service delivery andplanningWorks with Clinical Director to increase effectiveness of therapeutic interventionsPrepares written Discharge Summary and PlanApprises Clinical Director of workload, case activity, service barriers, and procedural problems in staff andone-on-one meetingsRequired Licenses/Certifications:QMHP CertificationCPR/First AidTOVAPhysical Demands and Work EnvironmentThe physical demands described herein are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee must occasionally lift and/or move up to 25 pounds. The employee will be required to complete a tuberculosis screening upon hire.
