Job DescriptionJob DescriptionPathWays has provided quality services to individuals with developmental disabilities and their families for over 50 years. Across Sullivan and Lower Grafton Counties, we serve individuals from birth through all stages of life.Full-Time | Hourly | Non-Exempt$21.00 – $23.00 per hour (commensurate with experience)Hybrid work option available after 90 daysAbout this Role:As a Family Services Case Manager (FSC), you’ll be a trusted advocate and the primary contact for individuals and families navigating services. You will coordinate evaluations and supports, connecting them with local and state resources to help them live full, meaningful lives.This role offers fulfilling work, a collaborative team environment, and the opportunity to make a lasting impact in the community.Responsibilities:Advocate for and support individuals and families in accessing community resourcesCoordinate assessments and evaluations (e.g., SIS, HRST) Facilitate planning meetings and develop Individual Service Agreements (ISA’s), Amendments, Action Plans, and other related documentsMaintain clear documentation and compliance with service plansBuild strong relationships with families, internal teams, and community partnersTravel throughout NH to meet with families and providers (mileage is reimbursed)Qualifications:2+ years of Case Management experience or closely related work Strong organizational skillsProficiency with documentation and MS Office (Word, Outlook, Excel)Excellent verbal written and interpersonal communication skillsWillingness and ability to travel within the state of NHPreferred:Bachelor’s degree preferred; Candidates with extensive relevant experience in case management or related areas are strongly encouraged to applyExperience with developmental disabilitiesBenefits:Generous PTO; start earning paid time off on your first day!
