New Hire Testimonial What Youll DoAs a Plan Developer youll:Advocate for and support individuals in making their own life choices and achieving personal goals.Develop and implement individualized Staff Action Plans (SAPs) aligned with each persons Life Plan.Participate in Life Plan meetings and collaborate with care managers and service providers.Promote self-advocacy independence and community inclusion.Maintain accurate documentation and communication with all stakeholders.Build trusting respectful relationships with individuals and their Circle of Support.Provide hands-on support as needed in accordance with the service plan.Uphold ethical and professional standards in line with the NADSP Code of Ethics RequirementsAssociates degree preferred1-2 years related experience and training requiredExceptional written communication and computer skillsA valid drivers license.Ability to be certified in all required trainings. ESSENTIAL DUTIES AND RESPONSIBILITIESDevelop a shared relationship with the people who choose Coarc services that supports the unique personal desires and needs of each person; provide support with decision-making problem-solving and providing encouragement to become as much of a self-advocate as possible.Assist the person to know their own personal strengths and worth; assist the person in sharing those personal traits with others.Responsible for the development and implementation of the Staff Action Plan process inclusive of participation in the Life Plan meetings for each assigned person; work with the person and his/her care manager to establish meaningful objectives to meet the valued outcomes for the service.Author Staff Action Plans or other service plans as required by regulation.Communicate with program staff and program management to ensure support needs are represented in Life Plan/Staff Action Plans.Review draft Life Plans to ensure accuracy.Maintain records and correspondence as required for Life Plan/Staff Action PlansAct as liaison to families care managers residential providers day service providers advocates and/or correspondents for those supported.Work in a professional and ethical manner; establish a trusting relationship with the people supported by being reliable and on time when providing supports and/or activities based on the persons stated valued outcomes and goals; always work towards assisting the person to achieve maximum success by developing a mutually respectful relationship with the person and their Circle of Supports.Provide coverage and hands on implementation of the Staff Action Plan or other service plan as required.
