Position Title: Resource SpecialistWork Location: Region A (Counties Preferred: Santa Rosa, Escambia, Okaloosa, Walton, Holmes, Washington, Bay, Jackson, Calhoun, Gulf, Liberty, Franklin, Wakulla, Leon, Gasden, Jefferson, Taylor, Madison, dixie)Assignment Duration: 3 MonthsWork Schedule: 8am-5pm, Monday - Friday (Weekend work may be needed during Hurricane Season for disaster outreach)Work Arrangement: Remote (Field visits)Position Summary:This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.Key Responsibilities:Compliance with care management policies/procedures including outreach and documentation expectations.Individuals will be audited on a monthly basis to ensure charts are in compliance.Qualification & Experience:Pediatric case management experienceExperience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.Health plan experienceFoster care and/or adoption experienceBachelor's DegreeMicrosoft Office- Outlook, Word, and ExcelRequired Skills/Experience: Preferred Skills/ Experience: 1.Pediatric case management experience1.Health plan experience2.Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.2.Foster care and/or adoption experience3. 3.
