Every other weekend coverage is required. Must have previous at home experience Location: 50 Water Street 5th floor NEW YORK NY 10004 Job Summary: The Adult Care Registered Nurse provides comprehensive nursing care to adult patients in their homes across the Bronx borough. This role requires strong independent assessment skills clinical experience across multiple acute care specialties and the ability to deliver safe patient-centered care in a home environment. The RN is responsible for coordinating care monitoring patient progress providing education and ensuring continuity of services. This is a 40-hour/week Monday Friday 9:00 AM 5:00 PM position with every other weekend coverage required. Responsibilities Provide skilled nursing care to adult patients in their homes performing assessments and monitoring clinical status. Evaluate home safety medication compliance vital signs and patient progress. Develop individualized care plans and coordinate with physicians therapists and interdisciplinary teams. Perform nursing interventions including wound care IV care injections catheter care and chronic disease monitoring. Educate patients and caregivers on medication management safety disease processes and self-care strategies. Document assessments interventions and patient updates in accordance with agency and regulatory standards. Recognize changes in condition and implement timely interventions. Maintain compliance with infection control and home health regulations. Travel throughout the Bronx borough to complete scheduled home visits. Collaborate closely with care coordinators to ensure appropriate follow-up and continuity of services. Requirements Minimum 2 years of acute care RN experience in one or more of the following: Medical/Surgical Maternal & Child Care Critical Care Pediatrics Current Home Health Adult Care experience Required Ability to work independently in non-hospital settings Strong clinical assessment documentation and communication skills Ability to travel throughout the Bronx borough for home visits Education: Bachelor of Science in Nursing (BSN) Required Certifications: AHA Basic Life Support (BLS) Required License: Active New York State RN License Required Facility Details Administrative Office: 50 Water Street 5th Floor New York NY 10004 Service Area: Bronx Borough home health visits Community-based care model focused on patient independence safety and holistic support Required Skills : Basic Qualification : Additional Skills : The candidate must be on HonorVets W2.
