Job Details Remote - Remote FL Fully Remote Full Time $80000.00 - $100000.00 Salary Up to 25% Health CareDescription Join Our Team!Bilingual Korean/English-speaking Registered Nurse (Telephonic RN Care Coordinator)Remote Full-Time $80000$100000 based on experience and locationilumed Value-Based Care / ACO REACH & MSSPilumed is seeking a bilingual BilingualKorean/English-speaking Registered Nurse to join our remote Care Management this role you will provide telephonic care coordination disease management support and patient education to Medicare beneficiaries helping them navigate their health with confidence while improving clinical outcomes.What Youll DoProvide telephonic care guidance education and support to beneficiaries and caregiversCollaborate with physicians and care teams to develop integrated care plansUse motivational interviewing to engage patients and identify care needsMeet care management KPIs for call volumes caseload and documentationIdentify SDOH needs and connect patients with appropriate resourcesEnsure compliance with HIPAA and regulatory standardsWhat You BringFluent in Korean & English (spoken and written)This role requires a Compact RN License as well as California and New York state RN licenses.If you dont have one or more of these yet thats ok were happy to cover the licensing costs for the right candidate.In addition:24 years of case management home health or similar RN experienceExperience with Medicare or Medicare Advantage preferredStrong critical thinking communication and time-management skillsProficiency with Microsoft Office and familiarity with EMRsWhy ilumedWe are transforming value-based care through personalized patient support data-driven insights and a mission to improve quality outcomes.
