This role is ideal for a nurse who is passionate about coordinating patient care supporting field staff and ensuring high-quality patient-centered services. As Case Manager you will play a vital role in fostering communication promoting patient well-being and ensuring compliance with regulatory standards and company policies.Responsibilities:Oversee and coordinate patient care for the Silver Spring office.Provide support guidance and communication to clinical staff to ensure high-quality care delivery.Collaborate with interdisciplinary teams to create individualized care plans that promote patient health independence and dignity.Monitor and evaluate patient outcomes implementing follow-up or care adjustments as needed.Ensure compliance with all regulatory and accreditation standards.Support staff through ongoing education training and professional development.Build strong relationships with patients families and community partners.Qualifications:Active RN license in Maryland.Minimum of 3 years of clinical experience in home health or a related healthcare setting.Experience in case management or care coordination preferred.Strong organizational communication and interpersonal skills.A passion for supporting staff and fostering a collaborative team culture.Required Experience:Manager Key Skills Hyperion,Acquisition,Asset,ABAP,Basic,Activex Employment Type : Full Time Experience: years Vacancy: 1
