If youre passionate about continuity of care deeper patient connections and a calmer more sustainable pace that prevents burnout youll thrive here!Responsibilities:Conduct comprehensive assessments of patient needs and develop individualized plans of care.Coordinate and manage a caseload of home health patients including skilled nursing physical therapy occupational therapy and speech-language pathology.Obtain and review physician orders referrals and medical records.Educate patients and families on disease management medication administration and self-care techniques.Monitor patient progress identify potential complications and adjust care plans as needed.Ensure compliance with all regulatory and accreditation standards.Maintain accurate and timely documentation in electronic health records.Collaborate effectively with physicians nurses therapists and other members of the healthcare team.FCS works to support our team members just as they support our patients.
