Job Title: RN Case Manager Location: Syracuse NY Assignment Duration: 13 weeks Schedule: MondayFriday 9:00 AM 5:30 PM Pay: $40 $58/hrJob Summary:Navitas Healthcare LLC is seeking RN Case Managerfor an exciting job in Syracuse NY.Responsibilities:Conduct comprehensive patient assessments and manage case management from admission through discharge.Perform MDS evaluations ensuring compliance with regulatory and hospital standards.Complete Utilization Review to verify appropriate patient status level of care and medical necessity.Participate in Clinical Documentation Improvement (CDI) to optimize accuracy of patient records for quality and reimbursement.Coordinate with interdisciplinary teams to develop and implement patient-centered care plans.Facilitate safe and timely discharge planning and care transitions.Document track and communicate patient information using EPIC EMR.Adhere to hospital policies best practices and regulatory guidelines.Qualifications:Active New York State RN License in good standing.Current BLS certification (or ability to recertify).Minimum 1.5 years of recent acute care experience within the last 2 years in a hospital of similar size or trauma level.Prior experience with MDS Utilization Review and acute care case management.Proficiency in EPIC Electronic Medical Records.Experience in hospitals of equal or larger size or trauma centers preferred.For more details contact at or Call / Text at .About Navitas Healthcare LLC certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical Clinical and Non-Clinical services to numerous hospitals.
